My name is Marta Costa Cardoso, I am Portuguese and a recently graduated (2017) medical doctor from Portugal. I have completed my medical studies - Integrated Master's degree in Medicine at the Faculty of Medicine of the University of Coimbra. I always felt the urge of doing something more than just ‘treating the disease’, I saw myself treating patients and their families over their lifespan, in a longitudinal perspective of Health Care. Thereby, I chose Family and General Practice (GP) specialty and I’m currently on the 3rd of this four-year specialisation training program. I work in a Portuguese Primary Health Care Centre at Coimbra - more precisely at USF Araceti - a Family Health Unit, of ACES Baixo Mondego - a Health Care Centre Group.

My name is Margarida Cepa and I am a fourth year specialisation trainee as General Practitioner at a Primary Care Unit. I am in my last year of specialisation in General Practice, graduating by October 2021. My education is focused on health and investigation. I first got a six-year degree in Pharmacy (2002), got a PhD in Pharmacy (2009) and afterwards I enrolled in Medicine degree which I finished in 2015. I worked as a Pharmacist both in Community and in a Central Hospital, as an Invited Asssistant Professor and also in a Health System Company, as a Health Content Specialist. I applied for specialisation in General Practice and I am now working in a primary care facility in a town in the Centre of Portugal. Now, I´m working closely with patients and I really take a lot of pleasure form my job.

Prof. Dr. Niels H. Chavannes MD, PhD, graduated in Medicine at Maastricht University in 1998. He combined his specialization as a Family Physician (2003) with several diagnostic and therapeutic studies in primary care, resulting in his 2005 PhD thesis: “Tracking and treating COPD in Primary Care: An integrated approach to diagnosis and therapy” at the CAPHRI Research Institute of Maastricht University, the Netherlands. In Rotterdam he was involved in setting up an innovative multidisciplinary health care center in a deprived area since 2003, and practised there as a Family Physician in an integrated primary healthcare team. In 2006 his team received the National Public Health Stimulation Award for the Kroonluchter COPD Project, implementing a highly successful integrated COPD management program. In 2008, this was followed by two years as a Consultant Family Physician at United Family Hospital in Shanghai, China, combined with an Assistant Professorship at Leiden University Medical Center. In 2010 he returned to the Netherlands as an Associate Professor, coordinating several (inter)national clinical research projects, and teaching on chronic disease management, eHealth and mHealth applicability, therapy adherence, and smoking cessation strategies. In 2015 he was appointed as a Full Professor of Primary Care Medicine, Strategic Chair of eHealth Applications in Disease Management, and in 2016 he became Head of Research at the Department of Public Health and Primary Care, Leiden University Medical Center. He is also appointed as Visiting Professor at the Respiratory Research Institute at Fudan University, Shanghai, China. In 2020 his Chair was continued as eHealth Applications in Population Health Management and he became Chair of the Theme Prevention and Lifestyle within the Research Council of LUMC. He has been supervising 32 PhD students, of whom 7 have graduated, two cum laude. His multidisciplinary team currently consists of 8 postdocs and 4 assistant professors with clinical, psychological, epidemiological and biomedical expertise, and several projects are embedded in the Medical Delta eHealth Institute, of which Chavannes is one of the founders. After working as a part-time Family Physician in Zeist, he now enjoys working at the Streetdoctor Group in Rotterdam, the Netherlands, taking care of the homeless and deprived at the inner city Pauluskerk. He is the Vice-Chair of the Dutch Asthma and COPD Advisory Group (CAHAG) and the National Advisor of the Dutch Action Program on Chronic Lung Diseases of the Lung Alliance Netherlands (LAN). In March 2018 he opened the National eHealth Living Lab (see www.nell.eu), a patient-centered multidisciplinary Open Science platform to scale up a large number of eHealth initiatives in an evidence-based manner, using innovative eHealth methodology and stimulating public-private partnerships.
