I am a Professor of Physiotherapy working in Christian Medical College and Hospital, Ludhiana, India, serving those with Neurological illness. Stroke is my field of expertise and passion. I head a small team of Neurophysiotherapists at our institute and am a telerehabilitation consultant. My research journey began almost a decade ago and it has been an exciting journey. Currently I also am part of the Lancet Commission (Technology in rehab workstream), Lancet Neurology Commission (Stroke Rehab Pillar), Project Officer for building capacity among stroke rehab professionals in South East Asia in collaboration with World Health Organization (SEAR) and the founder of the 'Global Consortium of Stroke Rehabilitation'.The consortium is currently working towards developing context and region specific Clinical Practice Guidelines for Stroke rehabilitation in Low-and-Middle-Income-Countries (LMICs) I work with Bio-engineers from India in designing and use of low-cost rehabilitation robotics and equipment in addition to academic responsibilities. Abstract

Background and Aims

Similar to other LMICs, there is a considerable need for improved neurorehabilitation services in India however the challenges in implementing the same are quite unique to this subcontinent. Aims: (1) To explore the challenges to implementing Physical Neurorehabilitation at in-hospital and community levels in India, (2) To summarize the unmet needs of successful physical neurorehabilitation in India, (3) To explore the potential models/ services that can enhance the delivery of physical Neurorehabilitation in India

Methods

Following the PRISMA guidelines for Scoping reviews we included 28 studies on a pre-determined selection criteria. We narratively synthesised data on the challenges and current needs of neurorehabilitation in India.

Results

Out of the 28 studies most studies were on Stroke rehabilitation. Current challenges and unmet needs were described under 4 categories (a) Rehabilitation (b) Rehabilitation provision, (c) Rehabilitation research and (d) Socioeconomic factors. We also propose 8 ‘Models of Care’ that have been adapted from previously studied models which demonstrate evidence of effectiveness and may prove to be potential solutions in filling existing gaps.

Conclusions

There is no one-size-fits-all solution to the diverse challenges and needs of neurorehabilitation delivery in India. Despite the unmet needs there are several alternative models of care which seem likely to be effective in future. Improving public awareness, building capacity among the existent professionals and pro-active involvement of local and national governments and other stakeholders are few of the fundamental measures towards the way forward.
