This presentation covers the challenges stroke patients face after discharge from the hospital, when and how the COMprehensive Post-Acute Stroke Services transitional care model provides direction during this vulnerable period, which patients benefit from transitional care, and where telehealth follow-up could address some of the barriers to transitional care.
