Session Description
Contemporary scholars define health related quality of life (HRQoL) as a multidimensional construct, involving an individual feeling of general well-being in the areas of "physical and psychosocial functioning". In the last ten years, HRQoL has become an important tool for assessing functional recovery in individuals with mental health and somatic conditions. In this symposium we will overview recent studies which deepen the understanding of functional outcomes of individuals with mental health and somatic conditions by investigating a set of cognitive, clinical and personality predictors and their impact on HRQoL.

In this presentation, the underlying mechanisms of health benefits and the risk of habitual behaviours such as the usage of internet and physical exercises are explored, considering their associations with the reward/motivation system. It highlights that several daily habits that are beneficial when undertaken normally may potentially lead to excessive behaviours. This is especially critical in the recent era, where technology like the internet has become mainstream despite the enormous addictive risk. The understanding of the mechanisms of behavioural addiction and optimal level of habitual behaviours for heightened Quality of Life (QOL) in mental health perspectives are explained by introducing several MRI studies. With the evolution of the world, more effective strategies for preventing and managing daily habits are in more demand than before, and the insights of this study are also valuable foundations for future research of QOL in mental health.

FACTORS ASSOCIATED WITH WORSENED PSORIASIS CLINICAL SYMPTOMS AND DISEASE-RELATED QUALITY OF LIFE DURING THE COVID-19 LOCKDOWN

Background: COVID-19 pandemic period bring lifestyle changes and had a strong impact on individuals with somatic problems psychological wellbeing and quality of life; and psoriasis was no exception.

Objective: In this cross-sectional study we evaluated the factors, contributing to the worsen outcomes in subjects with psoriasis during COVID-19 pandemic lockdown and to search for possible moderators of the relationship between psoriasis severity and disease specific quality of life (QoL).

Methods: Individuals, diagnosed with psoriasis were invited to fill anonymous online survey (January–April 2021, during second COVID-19 national lockdown) including sociodemographic, life-style factors, psoriasis-related clinical symptoms, their changes, Dermatology Life Quality Index (DLQI) and The Patients’ Health Questionnaire (PHQ).

Results. The final sample comprised 297 individuals with psoriasis. The strongest predictors of worsened psoriasis symptoms were the need for changes of psoriasis treatment OR = 2.73, 95% CI 1.37–5.44, p = 0.004 and the decreased income OR = 2.33, 95% CI 1.30–4.17, p = 0.004. The strongest predictor for QoL was male sex OR = 3.35, 95% CI 1.70–6.59, p < 0.001. Depression symptoms were significantly associated with psoriasis symptoms worsening OR = 1.10, 95% CI 1.05–1.14, p <0.001 and to the worse QoL -OR = 1.07, 95% CI 1.02–1.16, p = 0.008.

Conclusions. The strongest predictors for worsened psoriasis symptoms were the need for changes of psoriasis treatment and decreased income; the strongest predictor for QoL was male sex. Depressive symptoms have been found as possible moderators of the relationship between psoriasis severity and disease specific QoL during COVID-19 period.

PREDICTIVE VALUE OF BASELINE COGNITIVE FUNCTIONING ON HEALTH-RELATED QUALITY OF LIFE IN INDIVIDUALS WITH CORONARY ARTERY DISEASE

Background: Relationship between baseline measures of cognitive functioning and long-term health-related quality of life (HRQoL) with specification to Type D personality traits and sex has never been examined in the population of individuals with coronary artery disease (CAD).

Methods: This prospective observational cohort study was comprised of 864 participants (mean age 58 SD=9 years old, 74.0 % men) with CAD after acute coronary syndrome. Baseline characteristics included comprehensive cognitive testing, measures of socio-demographic and clinical factors, as well as psychological assessment scales, such as Type D personality scale and the Hospital Anxiety and Depression scale. Participants’ HRQoL was assessed with the Minnesota Living with Heart Failure Questionnaire conducted through phone interviews at baseline, every six months for up to two years, and after five years.

Results: Cognitive functioning was correlated with HRQoL at all time intervals over the five-year follow-up. In regard to sex and Type D personality, significant differences emerged in associations between impaired cognitive functioning at baseline and HRQoL measured over the period of five years. Men participants with characteristics of Type D personality were especially vulnerable for impaired cognitive functioning to affect five-year quality of life.

Conclusions: Men with CAD who obtained scores indicating characteristics of Type D personality were significantly more likely to have lower baseline cognitive functions and long term HRQoL outcome.

MENTAL WELL-BEING, EXERCISE ROUTINES, AND SUPPLEMENT USE DURING THE COVID-19 PANDEMIC: FROM THE PERSPECTIVE OF ADDICTION AND ENHANCEMENT

Objectives

Our studies with an online questionnaire and MRI imaging were performed to survey the neurobasis of excessive exercise, which is considered one of behavioral addiction.

Different sports disciplines were compared in psychological characteristics to investigate what is a the health-beneficial style of exercise, without leading to addictive problems is. Furthermore, MRI scans were performed on sports players.

Methods

The habit of exercise, use/non-use of IPED, and psychometric measures of Exercise Addiction Inventory (EAI), Appearance Anxiety Inventory (AAI), and Self-compassion Scale (SCC) were explored in a large sample (N= 2,295). MRI scans (7T-scanner) and psychometric measures were performed in those who have a habit of exercise such as cycling.

Results

The groups which are engaged in Weight Lifting and CrossFit were found to be more at risk of excessive exercising and more inclined to use the IPED. On the contrary, those who make walking a habit had low EAI scores as well as a low rate of IPED use. MRI scans of sports players have already been started to be performed and completed in several cases.

Conclusions

The kinds of sport that demand high-intensity functional training would have more risk of leading to excessive exercise and cross-addiction, while walking has low in them.

As a suggestion, the cultivation of self-compassion could play an important role in improving coping skills or reducing the risk of substance use disorder and act as a meaningful addition. Brain functional alterations in sports players will be introduced further as a preliminary data at the symposium.
