Cluster headache is still an enigma, and the related pain attacks are excrutiating and highly disabling. It is well defined primary headache and occur in either an episodic or chronic pattern. In episodic cluster headache the attacks occur in bouts separated by attack free remission periods lasting several months to years whereas there are no long lasting remssion periods in the chronic form. The clinical diagnosis should be fairly clear but still many patients are misdiagnosed and mistreated for years. The existing treatment possiblities can be quite effective but are still unspecific and hampered by side effects. Calcitonin gene-related peptide (CGRP) is highly involved in migraine and these new drugs targets have revolutionized migraine treatment. However, a similar dramatic effect in cluster headache is lacking and the role of CGRP in the context of such a cyclic disease pattern is unknown.
In this presentation existing knowledge about clinical presentation, prevalence, and burden in cluster headache will be presented along with the newest genetic and pathophysiological insights. The existing and future treatment possibilities will be discussed.

Through the course of my presentation I will be reviewing the evolution of our current understaning of headache assoicated with both acute COVID-19 illness, as well the persistence of headache that can occur for some in the post-COVID condition. I will be discussing prevalence, characteristics, postulated theories behind its occurence, and the potential impact that the pandemic condition itself has had on those with underlying primary headache disorders.
