P239 - Amniotic Epithelial Stem Cells or Adipose-derived Cells? A Comparative Evaluation in a Large Animal OA Model.
Abstract
Purpose
A challenge in osteoarthritis (OA) is to find a minimally invasive biological therapy able to contrast the progression of the pathology in an early phase, both on the inflammatory and on the structural fronts, to avoid or delay the need for prosthetic surgery. This study aims to evaluate and compare the effects of one intra-articular injection of three different biological treatments, autologous culture expanded adipose-derived mesenchymal stromal cells (ADSCs), autologous stromal vascular fraction (SVF) and allogenic culture expanded amniotic epithelial stem cells (AECs) in a large animal model of mild OA.
Methods and Materials
Twenty-four adult sheep underwent OA induction through bilateral lateral meniscectomy. After 6 weeks from meniscectomy, one injection for each group was performed. At 3 and 6 months from the treatments, results were analyzed through macroscopic, histological, histomorphometric, immunoenzymatic, and biochemical evaluations of the entire knee joint.
Results
All treatments showed better results than control (injection of NaCl). SVF showed superiority over the other ones, in terms of cartilage regeneration and in counteracting the inflammatory microenvironment and AECs showed superiority over the ADSCs. At 3 months, SVF significantly reduced articular damage score more than all the other treatments (p=0.04, p=0.012, p=0.04 for AECs, ADSCs and NaCl, respectively). AECs had better results than ADSCs (p=0.012) and NaCl (p=0.03), and ADSCs than NaCl (p=0.03). At 6 months, SVF induced significantly lower values than ADSCs (p=0.023) and NaCl (p=0.001). SVF showed more improvements, in some aspects, in comparison AECs in all the analyzed at 3 and 6 months.
Conclusion
SVF proved more favorable results versus the other biological options and control. SVF is easily obtained and rapidly used after the harvesting and showed a good healing potential. On the other hand, AECs cause no discomfort during tissue isolation, no ethical problems and offers good results for the treatment of OA joints.
