SE-022 - Presurgical Percutaneous Transhepatic Biliary Drainage in Patients with Resectable Perihilar Cholangiocarcinoma: An Analysis of Safety and Outcomes in a Single Specialized Institution.

Abstract

Purpose

To evaluate the safety and efficacy of preoperative percutaneous transhepatic biliary drainage (pPTBD) in patients with potentially resectable perihilar cholangiocarcinoma (prPHCC).

Material and methods

We retrospectively reviewed all patients with prPHCC who underwent pPTBD between February 2010 and July 2018 in our institution. Severe complications between pPTBD and surgery and in the 90-day period after surgery were analyzed, using Society of Interventional Radiology and Clavien-Dindo classification. Additionally technical, therapeutic success rates, conversion to EBD (endoscopic bile duct drainage) and clinical parameters were evaluated.

Results

A total of 29 patients with prPHCC who underwent pPTBD were included in the analysis. Six patients (20%) had at least one severe complication between pPTBD and surgery, and 16 (55%) in the 30-day period after surgery. Four patients (13%) died in the latter period. Technical and therapeutic success rates of pPTBD were 93%, each. No conversion to EBD was noted. The mean time between pPTBD and surgery was 18.5 days (95% CI: 14-23). Twenty-eight patients underwent surgery and 1 patient refused surgical treatment. Among them, 22 (79%) were treated with a partial hepatectomy, while 6 (21%) underwent a staging laparotomy due to unresectable stage of tumor detected during surgery.

Conclusion

In our experience, patients with prPHCC who underwent pPTBD had an acceptable severe complication rate, similar to that reported in the previous literatures. Adequate selection of the patients with prPHCC for pPTBD may improve therapeutic management of these patients.
