Professor Bruce Campbell is a consultant neurologist, Head of Stroke and Interim Head of Neurology, Royal Melbourne Hospital. He is a professorial fellow in the Melbourne Brain Centre @ RMH, Department of Medicine, University of Melbourne, an honorary professorial fellow at The Florey Institute of Neuroscience and Mental Health and a fellow of the Australian Academy of Health and Medical Science. His research interests are focused on the imaging and treatment of acute stroke and he was co-principal investigator and medical co-ordinator of the EXTEND-IA and EXTEND-IA TNK multi-centre randomised trials published in the New England Journal of Medicine in 2015 and 2018. He is Chair of Clinical Council and a director of the Australian Stroke Foundation and co-chairs the Australian stroke “living” guidelines working party.

The EXTEND, ECASS4 and EPITHET data indicate that perfusion mismatch selection can be used to identify patients who benefit from thrombolysis beyond 4.5h after onset. However, these trials preceded the evidence for thrombectomy >6h and so we lack data on how to best combine thrombolysis and thrombectomy beyond 4.5h for the 60-70% of patients in the EXTEND trial who had a thrombectomy-eligible large vessel occlusion. This lecture will discuss the evidence to date and ongoing trials such as TIMELESS and ETERNAL.
