Background : Setting up a new system is often challenging, particularly if it involves changing the existing routine that people have become used to. In MENA region, with population of more than 600 million and more than 17 countries with heterogeneous health care systems and limited resources in the majority of its parts, these challenges can be daunting.

In the light of cross-sectional analysis of background information on stroke, current treatment options, procedure reimbursement as well as individual hospital environment, e.g. space, staffing, teams, budget, and management.Taking into consideration MENA regional challenges in stroke health care systems and the shortage in different segments of the stroke chain of care resources.

Aim : This review sheds light on a general, comprehensive overview , on behalf of Middle East North Africa Stroke and Interventional Neurotherapies Organization (MENA-SINO) Group of experts , on each stage of the patient pathway and what to consider when improving an existing service or when setting up a new service from scratch.Shedding light on some models in setting National Intervention Programs for AIS which showed success and could be inspirational to other developing systems of care in the MENA region. Additionally from a comprehensive dissecting understanding of health care systems in the region it will describe regional fitting roadmaps for setting a Neurointrventional Program overcoming the regional based obstacles and for maximizing the stroke treatment benefit in the light of cost-effectiveness agenda for that part of the world.
