Bio Dr. Dileep R. Yavagal, MD, FAHA, FAAN, FSVIN is the Director of Interventional Neurology and Co-Director of Neuroendovascular Surgery at the University of Miami & Jackson Memorial Hospitals and Clinical Professor of Neurology and Neurosurgery at the University of Miami of Medicine. He is the founder and Global Co-Chair of the Mission Thrombectomy 2020+ Campaign. Dr. Yavagal is an international thought leader in endovascular therapy for ischemic and hemorrhagic stroke as well as a pioneer in the translation of intra-arterial delivery of cell therapy for stroke. He was the national Co-PI of the first US multicenter clinical trial of Intraarterial delivery of autologous bone marrow stem cells for ischemic stroke: RECOVER Stroke. Dr. Yavagal has received several state and federal research grants to study endovascular stemcell therapies for ischemic stroke using small and large animal models of stroke in his researchlaboratory. He is considered a pioneering researcher the field of intra-arterial delivery of stemcells in stroke therapy. In 2018, he appointed to lead the Neurological Cell Therapy Platform at the Interdisciplinary Stem Cell Institute at the University. Dr. Yavagal has also made significant contributions in the field of mechanical thrombectomy for stroke. He was on the on the steering committee of the SWIFT-Prime and MR RESCUE, both landmark randomized clinical trials of endovascular stroke therapy. He co-authored the landmark 2015 AHA Endovascular Stroke Therapy Guidelines as well asthe recent groundbreaking DAWN stroke trial in the New England Journal of Medicine. He has also co-authored the AHA Policy statement on Stroke Systems of Care published in May 2019 He is the founder and Past-President of the Society for Vascular and Interventional Neurology (SVIN).

Abstract

Background and Aims

Mechanical Thrombectomy has been established as a highly cost-effective therapy for large vessel occlusion stroke (LVOs) since 2015. However, access to MT for eligible LVOs patients is limited due to informational & diagnostic, geographical and financial barriers. MT 2020+ is a global stroke campaign aimed at accelerating access to mechanical thrombectomy (MT) using public health interventions (PHI) through a regional committees (RCs) global network.

Methods

MT2020+ conducted an electronic survey in 22 RCs of different income levels in from November 2020-February 2021 with a questionnaire that included MT eligible patients, centers, operators, procedures and reimbursement. MT access was defined as the proportion of estimated MT eligible patients receiving MT. Linear regression models were used with countries classified by World Bank income classification.

Results

For high, upper middle and lower middle income countries, MT access was 15.75 %, 3 % and 0.06 %, MT Centers per million population were 0.95, 0.53 and 0.08 and MT operators per million population were 1.38, 0.81 and 0.10, respectively.

On univariate analysis, MT access rate was significantly associated with: income class ( p <0.011), reimbursement for MT (p <0.001) and % of GDP spent on healthcare (p = 0.001). On multivariate linear regression models, the factors that independently predict MT access were World Band income class (p <0.029) and reimbursement (p= 0.002).

Conclusions

There is a vast disparity in MT access across the globe. Country income and existence of reimbursement for MT in a given country were independently predictive of MT access for LVOs. These findings help prioritize PHI to accelerate MT access.
