General practice
I’ve always combined my academic activities with working as a general practitioner until a few years ago. From October 2007 till November 2019, I’ve been head of the department of general practice (since 2012 general practice and elderly care medicine) at VU University medical center. Since January 2019, I’m head of the Division of Primary Care, Public and Occupational Health and Methodology of Amsterdam University Medical Centers (Amsterdam UMC). My main focus in research has been on medically unexplained symptoms, I’ve supervised many diagnostic and prognostic research projects, and a number of RCTs carried out in primary care, resulting in over 200 international peer reviewed papers. All our research is carried out within the Amsterdam Public Health research institute of Amsterdam UMC. I’ve contributed to, or chaired, a number of (multidisciplinary) guidelines and I have also chaired or participated in committees of the Dutch Health Council. I’ve written or contributed to over 40 books on various topics, both for health care professionals and lay people. In 2018, I chaired the committee who steered the process of redefining the core values and core tasks of Dutch family medicine. Since 2020 I'm chair of the supervisory board of the Dutch College of General Practitioners (NHG).

Session Description
Nowadays, both governments and doctors are paying a lot of attention to prevention in order to diminish the burden of disease and thus keep health care sustainable and affordable. The first question is on which preventive activities we should focus when providing care for older people, especially the very old. Should quaternary prevention, protecting people from overmedicalization, be our main goal? The downside of quaternary prevention might be that we take or are misinterpreted as taking a ‘laissez faire, laissez passer’ approach of not providing enough care for our patients. How do we achieve a balance and how do we discuss this with patients? Secondly, what are the ethical dilemmas in discussing advance care planning with patients who have been recently diagnosed with dementia, and in discussing this with their families? When and how should we proceed with advance care planning? Which issues should be covered, which issues should perhaps be avoided? In this Round Table we will explore both themes in-depth, after the two plenary speakers have provided us with food for thought. In small groups, we will have two rounds of discussion on an assignment for each theme which will be introduced by Dimity Pond.

Session Description
Personalised care is key to the profession of family physician. Recently, this term has also been used by experts from other areas of biomedical science, especially regarding genomics. But actually, family physicians are true experts in personalised care. This session will deal with the family medicine approach to personalised care. We will hear experts describing attitudes of family physicians from the Netherlands regarding personalised care for dementia. We will learn about experiences from China and the way patients there understand personalised care. The session will also include a study from the UK about the results of new practices in care for people with dementia. Finally, we will learn bout the factors that influence patients' decision of living with dementia at home in the Netherlands. I am confident that this will be a very interesting session and I am looking forward to your questions and a lively discussion.
