Background and aims:

Pertussis, or whooping cough, is especially serious in the first months of life. To improve prevention, vaccination is recommended during the third trimester of pregnancy. This way the newborn is protected by the cocoon immunization and by the transfer of pertussis-specific IgG antibodies through the placenta, until its own vaccination.

A systematic vaccination program for pregnant women in the third trimester was established in our health area since April 2016.

The aim of this study is to calculate association measures and the number needed to vaccinate to prevent a case of pertussis in infants younger than 6 months in the period before and after the systematic vaccination in pregnant women was established.

Methods:

A retrospective analytical observational cohort study was carried out between January 1, 2010 and December 31, 2019 in pediatric pertussis patients under 6 months, divided into “control group” (patients admitted until April 2016) and “intervention group” (admitted later). Associations measures and the number needed to vaccinate were calculated.

Results

The relative risk (RR) of having whooping cough in the intervention group compared to the control group is 0,37 (IC95 0,15-0,91). When doing a per-protocol analysis, the RR would be 0,17 (IC95 0,05-0,56). The number needed to vaccine during pregnancy to prevent a case of pertussis is 787.

Conclusions

Pertussis prevention strategy through vaccination during pregnancy reduces the risk of contracting it in infants less than 6 months. The number needed to vaccine to prevent a case of pertussis is 787.

The aim of this study is to compare the incidence, course and complications of pertussis, in infants younger than 6 months, before and after establishing a routine vaccination program for pregnant women in the third trimester.

Methods:

A retrospective analytical observational cohort study was carried out between January 1, 2010 and December 31, 2019 in pediatric pertussis patients, divided into “control group” (patients admitted until April 2016) and “intervention group” (admitted later). A descriptive study was carried out, and the incidence was calculated.

Results:

The incidence of pertussis in infants under 6 months decreased from the start of the vaccination (Figure 1).

Complications: 5 patients in the control group required transfer to intensive care unit, compared to none in the intervention group. There was one malignant pertussis in the control group.

The average of admission days in the control group was 9,53 and in the intervention group was 7.33 days.

Conclusions:

Pertussis prevention strategy through vaccination during pregnancy reduces the risk of contracting it and the severity of the clinical picture, in infants less than 6 months.
