Cord milking in preterm infants – benefits vs risks

Cord management, which enhances transfusion of placental blood into the baby, can lead to physiological transition at birth of preterm and term babies. Benefits for babies include a reduced mortality risk of 28%. Placental transfusion can be achieved by either waiting before clamping at cutting the cord for at least 1 minute or by gently stripping (milking) the umbilical cord towards the baby before clamping and cutting the cord. Variations of the techniques include starting resuscitation of the baby before clamping or clamping and cutting the cord and milking the umbilical stump towards the baby. Both main techniques have shown to increase survival of preterm infants at the same magnitude in recently published meta-analysis. Discussions are ongoing about which technique to best use in what kind of delivery scenarios. The recently published eveidence about risks and benefits of the available techniques will be presented and discussed.
